1. In the past 2 years, have you been diagnosed with, hospitalized for or received care other than medication for:

  • Diabetic coma or insulin shock?
  • Cancer (other than basal cell carcinoma)?
  • Cerebrovascular accident (stroke)?
  • Chronic respiratory illness for which oxygen is administered?
  • Chronic kidney or liver disease?
  • Angina, heart attack, cardiac arrest (heart fails to pump blood) or cardiomyopathy (heart muscle disorder)?
  • Organ or bone marrow transplant?

2. Are you currently awaiting treatment or the results of a test for one of the conditions listed in Question 1?

3. Have you ever been treated for or do you currently have:

  • Alzheimer’s or dementia?
  • Kidney failure?
  • Liver cirrhosis?
  • Lou Gehrig’s disease? 
  • AIDS or an HIV infection?

4. Are you:

  • Confined to a bed?
  • Hospitalized?
  • Living in a medical care or long-term care facility?
  • Unable to complete basic daily tasks on your own such as feeding, dressing and washing yourself, going to the bathroom, moving from your bed to a chair or controlling your bladder and bowel movements?
